Information to read, if you are asked to complete an Online Self-Assessment for your Carers Assessment
Any information you enter into this online form will be sent, once fully submitted, to Enfield Carers Centre, via the email address assessmentsadmin@enfieldcarers.org. This email can only be accessed by the Centre’s Assessment Team Administrator or their direct line management. The information you provide will be used to create a baseline assessment, about how your caring role impacts upon key wellbeing areas that are sometimes difficult to manage, when balancing the responsibilities of providing care.
A carers assessment is for adult carers, over the age of 18, providing support to another adult, considered to have Ordinary Residence in Enfield. Which in most cases just means that they are living in Enfield.
If, when answering questions in this form, you are asked to contact the Centre this might mean that you don’t meet the criteria to benefit from a Carers Assessment, and we would like to speak with you to see what support would be bests for you. For example, if you are a parent carer or young carer, there would be different options for support available, so we would welcome the opportunity to discuss this and assist you to access.
Once Enfield Carers Centre have received your completed form the information contained will be shared with Enfield Local Authority. As we would open a carer’s file on the confidential Social Care Database, (Eclipse), and create a Carers Assessment within this using your submitted information you have provided. This is a requirement for all Carer Assessments that are completed under Enfield Local Authority’s statutory remit, even if provided in the form of a self-assessment.
None of your information would be shared with any other 3rd party, without your permission and no actions would be taken, unless discussed and agreed with you first. The only exception would be if there could be potential harm or abuse caused to you or others if information isn’t shared/acted upon. The process is confidential, and any information provided would not be shared with the person you care for, and you both would have separate files on Eclipse, so if they asked to look at their file, this would not result in sharing any information you had submitted with them. We understand that caring can be personal and tough at times and appreciate your honest views about your caring situation and how they impact upon you.
The information you provide will allow us to evaluate how caring is impacting on you and to consider the statutory, and Centre, services that are most relevant in supporting your ongoing wellbeing.
